To break cyclic import, this patch makes `GhcPkg.hs` independent from `Monad.hs`(refactoring for #244)

This removes `ghcPkgList`, `ghcPkgListEx` and `ghcPkgDbOpt`. I'm not sure
this is a right way to do.
